Enhanced Care Management (ECM)

ECM provides whole-person care coordination for individuals with complex needs. Services can be delivered wherever support is most effective, including homes, schools, community settings, via telehealth, or in the Wellness Center.

ECM helps clients connect the dots between mental health care, physical health care, and community resources. Care managers work closely with individuals and families to reduce gaps in care and support follow-through across systems.

ECM support may include:
Coordinating care between providers and service systems
Helping schedule and attend important appointments when appropriate
Supporting care transitions and changes in needs over time

Connecting individuals and families to community-based resources

Ensuring care plans reflect the client’s priorities and goals
ECM works alongside therapy, medication support, school-based services, and other SCCS programs to help care feel coordinated rather than fragmented.

Medication Support Services (MSS)

Medication Support Services at SCCS include psychiatric evaluation, medication management, and ongoing monitoring. Our prescribers collaborate closely with therapists, ECM, and other team members to ensure medications are integrated into a comprehensive treatment plan. We work with adults, teens, and children when medication is appropriate as part of their care.
